I love the D200, possibly to an inappropriate extent. However, if I were purchasing again, I would seriously evaluate buying the D80 and spending my savings on more glass. And if the D300 were in the picture, I would definitely purchase either the D300 or the D80, not the D200. Love the 50 1.4, love the 18-200. I have the SB-800, but would probably buy the SB-600 instead, because most of the advantage seems to be in complicated multi-flash situations that I'm unlikely to set up. Regarding the bag, I've found that I like my backpack-style Crumpler, or my Lowepro Toploader 70AW (depending on what I'm doing) better than my shoulder bag. But, you can never have too many bags.
